Waive Form Hugo leite BJJ

1. The student acknowledge that have voluntarily agreed to participate in grappling/martial arts practice and instruction at Hugo Leite BJJ, located at 2400 s hwy 27 unit 4109 Clermont, Florida

2. The student understand that this type of sport is inherently dangerous regardless of level of supervision. Injuries may occur due to physical situations including a slippery floor surface, an overzealous partner, or a combination of any number of circumstances. It is clear that is voluntary participation in this activity will expose me to risk of injury, and that any amount of time spent engaging in this activity only makes it more likely that injuries will occur at some time.

3. THE STUDENT IS VONLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH KNOWLEDGE OF THE DANGEROUS INVOLVED AND HEREBY AGREE TO ACCEPT ALL RISKS OF INJURY OR DEATH.

4. As consideration for being permitted to engage in these activities, and to use the facilities provided by Hugo Leite BJJ, the student hereby agree that he/she will not make a demand, a claim against, or sue Hugo Leite BJJ, its employees, or authorized agents for any injury or damage resulting from negligence or other acts, howsoever caused as a result of his/her participation in martial arts/grappling. The student hereby release Hugo Leite BJJ, its employees, and authorized agents from all actions, claims, and demands that I now have or may hereafter have for injury or damage resulting from my participation in martial arts/grappling.
